Replacement of a Lost or Damaged Visa

The Visa Office may reissue a lost or damaged visa ONLY if the:

  • visa is a E, I, H, L, O, or P visa, AND
  • visa was revalidated (or reissued) in the Visa Office, in Washington within the last 12 months, AND
  • applicant includes a copy of a police report or evidence substantiating loss, AND
  • applicant submits all the necessary documentation and fees as required for a new visa.
