All waiver applications are subject to a processing fee of US$230.00 per Federal Register, Vol. 67, Thursday, March 28, 2002. The fee is the responsibility of the exchange visitor who is subject to the two-year home residence requirement of section 212(e) of the Immigration and Nationality Act, as amended.

-- Waiver case number, if known
-- Name
-- Date and place of birth
-- Social security number

Other issues

-- Personal checks are NOT accepted.

-- Remittances must be drawn on a bank or other institution located in the U.S. and made payable in U.S. currency to the U.S. Department of State.

-- If the applicant resides outside the United States at the time of application, remittance may be made by bank international money order of foreign draft drawn on an institution in the U.S. and made payable to the U.S. Department of State in U.S. currency.
